Health Homes provide comprehensive care coordination and care management services for Medicaid patients with complex combinations of chronic conditions and behavioral health disorders. Once enrolled, the program provides each member with a dedicated care manager to provide intensive care management and help members navigate the medical, mental health, substance use and social services systems. We were unfamiliar with everything hospice offers but Peter quickly educated us. After our first phone call he responded immediately and had a medical bed delivered to us within hours of first speaking to him. Additionally, he arranged for our loved one to be brought home from the hospital in an ambulance the next day and had a nurse waiting when she arrived home.
The nurse, Jennifer, has been incredible getting all the meds coordinated and educating the caregivers on how to manage everything.
